"Itmachi gavra v'itamchi kamia": Expert person - in all his amulets forever, as he became expert in three types of amulets. (Babylonian Talmud, Shabbat 61b) 
Lengthy amulet [approximately 74 words] on parchment, entirely handwritten by the greatest Kabbalist of our generation, Rabbi Yitzchak Kadouri. It is exciting to see how Rabbi Kadouri, as a son speaking before his father, entreats the Master of the world, explicitly expressing his requests for the subject of the amulet. As has already been stated: "A tzaddik decrees and the Holy One fulfills."
Amulets are usually written for specific needs or requests. This amulet before us is meant to be effective for all the important things in life: Fear of Heav-n, health, sustenance, graceful bearing, annulling the evil eye, and more.
Text of the amulet: "May it be Your Will, our G-d and G-d of our fathers, that for the sake of Your Mercy and Lovingkindness ... You send a complete recovery, recovery of the soul and recovery of the body, and equanimity for fear of Heav-n. May he be successful and benefit, and find grace and lovingkindness and mercy in the eyes of all who see him. May all the witchcraft, spells, and evil eye upon him be annulled for the sake of Your Holy Names ... Amen, may it be His will." Rabbi Kadouri then adds Holy Names, and in distinction, names of demons; so that the amulet will be effective.

The Kabbalist Rabbi Yitzchak Kadouri [1899-2006] knew and even saw the Ben Ish Chai. He studied Torah and kabbalah with the sages of Baval and of Jerusalem, where he ascended in 1922. In his first years in Jerusalem, he earned his livelihood binding books, and studied in the Beit El and Porat Yosef yeshivahs in the Old City. Over the years, his greatness in hidden wisdom became known, and many made their way to his home to ask for his blessing and counsel, and to receive amulets for salvation. He passed away in 2006 at the advanced age of over 100 years, and tens of thousands of Jews from across the Jewish spectrum attended his funeral.
Rabbi Kadouri is considered the highest authority on amulets in recent generations, and his amulets are known for their beneficial effectiveness. He authored the comprehensive book Pitchei Olam - a five-volume work on amulets, segulahs, and prayers. Regarding him and those like him, the gemara states: "Itmachi gavra v'itamchi kamia: amulets he writes will be effective forever." Rash"i comments: A person for whom three amulets have proven effective three times - It is guaranteed that all his amulets will effect their desired action.
