ברכת שנה טובה, בדפוס יפה, מודפסת בדיו בצבע זהב, עם ציורי מקומות קדושים, נדפסה בירושלים, בדפוס לונץ. תר"ם [1880]. בערך.
במרכז התעודה הנאה כיתוב ארוך המתאר את עזרתו הרבה של ד"ר אליעזר הלוי לטובת אנשי ירושלים.
בדף צמוד, מכתב ארוך במיוחד מרבי יהודה ליבוש ברנשטיין מכולל ווארשא, שנכתב סמוך לאחר פטירת משה מונטיפיורי, עם בקשה ממזכירו ויד ימינו של משה מונטיפיורי זללה"ה לסיוע כספי.
ד"ר אלעזר הלוי היה מזכירו ויד ימינו של משה מונטיפיורי במשך כיובל שנים.
